• Form I-102 Application for Replacement/Initial Noimmigrant Arrival-Departure Document
• Original I-94, I-94W, or I-95 arrival/departure record for each person included in your application if the card was damaged or contains incorrect information.
• Written statement explaining:
- the reasons for your request;
- why your extended stay would be temporary, including what arrangements you have made to leave the United States; and
- any effect the extended stay may have on you foreign employment or residency.
Fees
- Fees $320.00 – payable to US CIS*
*Access Legal charges $100.00 to complete, review, and submit your request to Replace or receive and I-94 Nonimmigrant Arrival-Departure Record. This fee includes assistance with any additional information that may be requested by US CIS in connection with your petition.
