Replacement of Citizenship Document
Form N-565 is used to request one of the following documents:
- Certificate of Citizenship;
- Certificate of Naturalization;
- Certificate of Repatriation;
- Declaration of Intention’
- a special Certificate of Naturalization to obtain recognition of the U.S. Citizenship by a foreign country.
You may request a Replacement if one the following reasons apply :
- lost, stolen, or destroyed certificate;
- mutilated or damaged certificate;
- name change of the citizen;
- certificate or declaration is incorrect.

Documentation Required
- Form N-565 Application for Replacement Naturalization/Citizenship Document
- Copy of the original certificate (if you applying based on lost, stolen, destroyed and have a copy in your possession).
- Original certificate (if applying based on name change, damaged or incorrect certificate).
- Name Change Decree Written if applying based on your name change.
- 2 color passport photographs taken within 30 days of your application.

Filing Fees
- Fees $345.00 – payable to US CIS*
