Application for Certification of Citizenship

• The child is regularly residing in the United States.
• The child has been lawfully admitted for permanent residence.
• The child has not yet reached his or her 18th birthday at the time of fulfilling all of the requirements for citizenship.
• If a biological child, the child is legitimate or was legitimated, while in the legal custody of the legitimating parent(s), prior to reaching his or her 16th birthday or
• If an adopted child, the child has been finally adopted and immigrated based on an approved I-600 or the child fulfilled the 2 –year legal custody, 2-years joint residence requirements.

• 3 photos of the individual for whom U.S. Certificate is requested;
  
• Copy of a Permanent Resident Card (Green Card) front and back;
  
• Parent (s)’ proves of citizenship, i.e. U.S. Naturalization Certificate and/or U.S. Passport and/or Parent(s)’ Birth Certificate;
  
• Child’s or applicant’s Birth Certificate;
  
• Adoption papers if adoption took place;

• $255.00 or $215.00 – if filing on behalf of an adopted minor child and
• $70.00 – for all applicants who is over 14 years of age at the time of filing
